Patient Forms, Map and Directions

Your first visit to OUR OFFICE involves a few important steps so that we can get to know you.   please read through this page TO FIND all the practical information you WILL need such as a map and directions to our office, TREATMENT EXPECTATIONS, practice hours, payment policies and more.   You can also save some time by printing out and completing the patient forms in advance of your appointment.

WELCOME TO ALL NEW PATIENTS!  We are so happy you have decided to visit our office for your dental needs and hope you leave feeling very satisfied and comfortable with us.

Here are some important things to remember for your initial visit:

*Arrive with ample time to complete or update any health or contact information with us.  This must be done yearly at a minimum.  Also, the HIPAA Privacy forms must be signed so you are confident we follow their privacy guidelines on keeping your treatment confidential.  Another important part of your visit to us will be communicating your health history and current medications.  Please take a moment yearly to update this (or sooner, if changes occur) so we are aware of your medical conditions as they can greatly affect your dental health and our treatment recommendations for you.  If you have had any type of joint replacement, heart or certain other surgeries, your physician or surgeon may require you to take a pre-medication before dental visits.  It is very important to check with your doctor and have them write the initial prescription for you and your specific medication needs.

*New adult patients will always receive a comprehensive examination with our dentist and his assistant.  This allows us to record all previous dental work you have had completed, review any concerns or expectations, and take any needed dental x-rays.  We highly recommend receiving a full set of digital x-rays which we will rely on in future visits and treatment and are valid for 3-5 years.  The four "bitewing" x-rays are taken once yearly by the hygienist for cavity detection between teeth.  These x-rays and their frequency have been set as a basic standard of care by the American Dental Association and are supported by our office.  Routine dental cleanings are recommended every six months unless specific or individual needs are detected.

*If you are registering as a new patient that is visiting us for a specific problem or tooth, a "problem-focused" exam will be scheduled just for that purpose.  The dentist will use this visit to examine the conditions and make any diagnosis and treatment options.  A new patient comprehensive exam can then be scheduled at a later date with a dental cleaning to follow.

*It is customary to pay for all dental services at the time of your visit.  If you are using dental benefits, we can review your coverage guidelines with you and calculate patient estimates using that information.  Most insurance companies will pay us directly however, some insurance companies pay the patient directly. In this situation all services must be paid at the time of the patient's visit. As a courtesy, we will file all dental claims for our patients and help maximize benefits provided by your insurance company.  Please keep in mind that while we try and work with an insurance company's benefit package, these can change without our knowledge and a patient's treatment needs can also change.  Ultimately, the patient is responsible for any dental treatment provided to them.
